Why Every Vegan Needs a B12 Supplement
This is one area in plant-based nutrition where the science is unambiguous: there are no reliable plant food sources of vitamin B12, and every vegan who is not consistently supplementing is at meaningful risk of deficiency.
That is not a fringe position or supplement marketing. It is the consensus of every major nutrition and vegan organisation the Vegan Society, Vegan Health, the British Dietetic Association and it is backed by decades of clinical evidence. B12 is the one nutrient that a well-planned wholefood plant-based diet, however varied and nutritious it might otherwise be, cannot provide reliably.

What Vitamin B12 Does
Vitamin B12 also called cobalamin is an essential water-soluble vitamin involved in some of the body's most fundamental biochemical processes. It cannot be produced by plants or animals; it is synthesised exclusively by certain bacteria and archaea. Animals accumulate B12 in their tissues by consuming food or soil containing these bacteria, which is how it enters the human food chain through meat, fish, dairy, and eggs.

Why B12 Deficiency in Vegans Is a Genuine Risk
B12 is stored in the liver, and the body's stores are substantial which is why deficiency typically takes years to develop after someone stops consuming reliable B12 sources. This is one of the reasons it is so easy to miss: a new vegan may feel entirely well for two, three, even five years before running into problems. By which point, tissue stores are genuinely depleted and functional deficiency is under way.
The scale of the problem in unsupplemented vegans is well established. A 2019 cross-sectional study of 151 adult vegans found that those not using B12 supplements had significantly lower serum cobalamin and holotranscobalamin levels, and were at 4.4 times greater risk of low plasma cobalamin and over 19 times greater risk of low holotranscobalamin compared to those who supplemented regularly. What B12 Deficiency Actually Looks Like
B12 deficiency exists on a spectrum from marginal depletion which may produce no obvious symptoms to severe deficiency with potentially irreversible consequences. The progression is gradual, which makes it deceptively easy to dismiss early warning signs.

Why homocysteine matters:
B12 is a cofactor for the enzyme methionine synthase, which converts homocysteine to methionine. When B12 is insufficient, homocysteine accumulates in the blood. None of this is to suggest that B12 supplementation prevents dementia or heart disease those are not authorised claims and the evidence is more complex. What it does illustrate is why adequate B12 status has significance well beyond avoiding anaemia.
The Plant B12 Myth: Spirulina, Nori, and Tempeh
This section matters because confusion here leads people to believe they are protected when they are not.
The following foods are commonly claimed as vegan B12 sources. The science does not support these claims:
Spirulina: Spirulina does contain corrinoid compounds molecules that measure as B12 in standard laboratory assays. But approximately 80% of the B12 measured in spirulina is pseudovitamin B12, which is not bioavailable in humans. The remaining fraction competes with genuine B12 at absorption sites. One published study found that B12 activity actually decreased in people fed a combination of spirulina and nori. Studies on populations using spirulina as a B12 source show deteriorating, not improving, B12 status. Spirulina is valuable as a protein and antioxidant source. It is not a B12 source.
Dried nori: Some forms of seaweed, including purple laver (nori), do contain measurable genuine B12. However, the amounts are variable, the content depends on how and where it was grown, and the quantities consumed in typical use are insufficient to maintain B12 status. Direct studies of vegans consuming nori as a B12 source have not consistently shown it to prevent deficiency.
Tempeh and fermented foods: The B12 content of tempeh and other fermented foods is highly variable ranging from near-undetectable to meaningful amounts depending on the bacteria present during fermentation. These are not controlled or standardised, and cannot be relied upon as a consistent dietary source.
Unwashed organic vegetables: Sometimes cited because B12-producing bacteria exist in soil. In modern food production, washing and hygiene practices mean any B12 from soil contamination is negligible and inconsistent.
The key test for a B12 source as the Vegan Society and vegan nutrition researchers consistently state is not whether B12 has been detected in a food in a laboratory, but whether consuming that food consistently prevents and corrects deficiency in human subjects. None of the above pass that test. Only reliably fortified foods and direct supplements do.
Fortified Foods: Useful, but Insufficient on Their Own
Some plant-based milks, fortified breakfast cereals, nutritional yeast (when B12-fortified check the label, as not all are), and certain meat alternatives are fortified with B12. These can contribute meaningfully to intake and are worth consuming regularly.
However, they are not sufficient as a sole strategy for three reasons. First, the amounts per serving are often modest and variable between products. How Much B12 Do Vegans Need?
EFSA sets an adequate intake (AI) of 4ยตg of vitamin B12 per day for adults. An expert consensus reported in the research literature indicates that 4โ20ยตg per day is adequate to prevent deficiency in healthy individuals with normal absorption.
However, B12 absorption is not straightforwardly linear. At low doses (around 1โ2ยตg), the gut absorbs roughly 50% of the dose via intrinsic factor the protein produced by stomach cells that binds B12 for active absorption. Methylcobalamin vs Cyanocobalamin
